The program provides personalized digital care plans, symptom monitoring, and timely support for patients to stay on track during cancer care treatment while at home
Markham, ON – February 4, 2025 – Oak Valley Health is excited to announce the expansion of its partnership with SeamlessMD to launch a new digital care journey for oncology patients. This initiative builds on the hospital’s ongoing success with the SeamlessMD platform, which has been instrumental in improving patient outcomes for patients undergoing surgery at Oak Valley Health since 2023.
The SeamlessMD Oncology program, which launched in October 2024, provides personalized digital support for patients undergoing chemotherapy, immunotherapy, radiation therapy, and survivorship. Patients can access the program on their smartphones, tablets, or computers, enabling them to stay informed with bite-sized education and to-dos, monitor symptoms and side effects, and receive timely support throughout their cancer journey. Since its launch, over 100 oncology patients have already been enrolled in the program.

By extending this digital care platform to oncology, we’re ensuring that every patient undergoing cancer treatment is equipped and empowered with the tools and education they need to actively manage their health, even between visits with their care team.”
The SeamlessMD Oncology program offers a range of features designed to enhance the patient experience, including:
- Personalized Education: Interactive, evidence-based care plans tailored to each patient’s treatment protocol, covering topics such as such as potential tests, managing treatment side effects, available resources, and post-treatment survivorship.
- Remote Monitoring and Instant Feedback: Daily health checks and symptom tracking for patients to report symptoms such as nausea, pain, and fatigue, to get immediate feedback on progress and what to do next. This also allows care teams to intervene early, reducing the risk of complications and hospital visits.
- Self-Care Tools: Access to a daily to-do list and digital library of resources, tailored for each journey, to help patients build confidence, manage side effects, and reduce anxiety.
Dr. Mateya Trinkaus, Medical Oncologist at Oak Valley Health, emphasized the importance of early intervention: “This program allows us to proactively monitor patients’ symptoms and respond quickly if issues arise. The ability to address problems early can significantly improve outcomes and a patient’s quality of life during treatment.”
Patients already enrolled in the program have expressed gratitude for the support it provides. Patients have reported that the program offers great reminders and educational resources that helped them feel more confident about managing their treatment at home.

Since February 2023, over 3,000 patients have benefitted from SeamlessMD’s care programs at Oak Valley Health, which demonstrated significant reductions in length of stay and readmissions and 97% of patients report feeling “more prepared” thanks to the app.
The SeamlessMD Oncology program is now available to patients undergoing active cancer treatment at Oak Valley Health.
