Remote Patient Monitoring Reduces ED visits and Readmissions for Transcatheter Aortic Valve Replacement Procedures
Montreal and Toronto, Canada – February 7, 2023 – The Montreal Heart Institute, the flagship of cardiovascular medicine in Quebec, a recognized research leader in Canada and around the world, and a true spearhead in cutting-edge cardiovascular medicine today announced the expansion of SeamlessMD, a leading Digital Care Journey platform, to their Transcatheter Aortic Valve Implantation (TAVI), Transcatheter Mitral Valve Replacement (TMVR) and other mitral and tricuspid procedures. The Institute first launched SeamlessMD Digital Care Journeys for Open Heart Surgery to optimize patient engagement and operational efficiencies. Building upon their digital patient engagement strategy, now patients undergoing TAVI, TMVR, Mitral and Tricuspid procedures can access SeamlessMD to be guided step-by-step from pre-surgical preparation through post-surgical recovery via personalized evidence-based education, progress-tracking, and post-op symptom monitoring using their own smart device or computer. The program is available in English and French.
“Introducing SeamlessMD into our TAVI program enabled us to reduce 30-day ED visits by 50.1% and reduce 30-day hospital readmissions by 33.5% among the patients using the application,” said Dr. Walid Ben Ali, Co-Director of the Transcatheter Valve Therapy Program at The Montreal Heart Institute, whose team recently published these results in the Journal of the American College of Cardiology. “Deploying a digital patient engagement solution optimized the clinical trajectory, achieving a median readmission rate lower than the Canadian national rate (7.9% vs 9.4%). The use of digital applications like SeamlessMD is an effective way to remotely monitor patients, digitally collect Patient-Reported Outcomes (PROs), and track symptoms in real-time.”
With the SeamlessMD platform, The Montreal Heart Institute can engage, connect and remotely monitor patients by delivering consistent evidence-based education in an interactive and patient-friendly way that enables patients to actively participate in their own care before, during, and after cardiac procedures. While recovering at home, patients can self-report data such as pain scores, symptoms, and share incision photos through the platform. Meanwhile, providers can monitor patients on dashboards allowing for personalized care recommendations and faster intervention from the surgical care team.
“It is evident from the data that the use of SeamlessMD in our TAVI Program improved patient quality and clinical outcomes. As a result, we’re excited to expand the use of SeamlessMD to patients undergoing TMVR and Mitral procedures,” explained Dr. Anita Asgar, Co-Director of the Transcatheter Valve Therapy Program, The Montreal Heart Institute. “Cardiac procedures can be daunting; our goal is to improve the patient’s overall experience by reducing their anxiety and enhancing their recovery by making sure they have easy access to educational materials during their entire episode of care.”
“Patient-driven care management has emerged as an essential component of modern healthcare as it promotes the patient’s active participation in their care plan and enables patients to self-manage their care journey,” said Dr. Joshua Liu, CEO and Co-founder, SeamlessMD. “We’re excited to expand our long-standing partnership with The Montreal Heart Institute as they continue to drive innovation forward with the delivery of convenient, efficient, and effective digital healthcare.”
In 2022, SeamlessMD was recognized as the CAN Health Network Company of the Year and named by AVIA Connect as a Top 5 Remote Monitoring Companies as well as one of the Top Patient Education companies.
