New Case Study: Prairie Heart Institute

We are excited to share exciting new results with Prairie Heart Institute, part of Hospital Sisters Health System, a 15-hospital system serving patients in Illinois and Wisconsin. Above is a video story developed in partnership with PHI, and below is a downloadable Case Study.

Prairie Heart Institute (PHI) is the largest cardiac center in Illinois. PHI identified several challenges in its transition to value based care, including variability in protocols, gaps in patient education materials, lack of real-time monitoring for patient compliance, and increased cost due to higher than expected length of stay, readmissions and discharge to skilled nursing facilities (SNF).

To address these challenges, PHI partnered with SeamlessMD and achieved the following results:

  • ↓ 1.0 days length of stay (LOS)
  • ↓ 45% 30-day readmissions
  • ↓ 64% discharged to Skilled Nursing Facilities
  • Epic EHR integration
  • Improved patient experience across cardiac surgery and cardiac electrophysiology
