79-Year-Old Crohn's Patient Empowered With SeamlessMD

The following is a patient success story by the team at Oak Valley Health:

Nora has lived with Crohn’s disease for the last almost 50 years of her life. This meant having to be mindful of her diet, be on medications, and endure a bowel resection, a hysterectomy, and several colonoscopies over the span of her diagnosis.

Nora is no stranger to procedures, and they have been a regular part of maintaining her health while living with Crohn’s. With her last colonoscopy at Markham Stouffville Hospital, Nora was introduced to an application called SeamlessMD, a program that would help her prepare for and recover from her procedure, via a personal mobile device.

At 79 years old, Nora admitted to not being very technologically savvy, but was open to giving SeamlessMD a try. She appreciated having all of the information upfront and on hand, and found the app user-friendly and straightforward to use.

“I liked that it was very simple and direct, with yes or no options,” says Nora. “It was very easy to navigate.”

SeamlessMD provides digital patient engagement and remote monitoring. The platform is key to improving patient experiences, post-procedure outcomes, and enhances the overall experience.

From their own devices (e.g., phone, tablet, computer, etc.) patients and their families now have access to:

  • Customized reminders and pre/post procedure to-do’s
  • A library of educational materials covering clinical and non-clinical topics
  • Health tracking and tailored feedback post-discharge (reporting symptoms/concerns they may be experiencing, etc.)

Our patients using SeamlessMD feel more supported in preparing for their scope procedure and feel less anxious.

To date, we have enrolled over 600 patients in the program since its launch in November 2023, and have opened up the program to accommodate additional patients who wish to take their health care into their own hands.
