November 1, 2019 – Roslyn, NY – St. Francis Hospital, The Heart Center® has partnered with SeamlessMD, a platform for patient engagement, remote monitoring, and virtual care. It is now using the platform for patients undergoing open-heart surgery and transcatheter aortic valve replacement (TAVR) procedures. St. Francis launched the initiative through its affiliation with Catholic Health Services of Long Island (CHSLI) - the first health system in New York to use SeamlessMD’s digital patient engagement platform, and the first in North America to use the technology for TAVR.

The platform is designed to empower patients and improve care through the course of their treatment. Patients enrolled on SeamlessMD are provided with step-by-step care pathways (education, reminders, daily milestone tracking) and can track their progress (e.g. pain levels, temperature) via their smartphones, tablets or computers before, during, and after hospitalization. Patients also receive personalized, automated feedback on how to self-manage pre- and post-procedure concerns based on their own inputs. Clinicians have access to alerts and dashboards to monitor patients remotely and address patient concerns earlier, leading to better patient experiences and health outcomes.

“This allows us to stay connected with patients and families beyond the four walls of the hospital while improving health outcomes. It also empowers our providers to deliver more coordinated care,” said Charles Lucore, M.D., MBA, President of St. Francis Hospital. “Our partnership with SeamlessMD supports our commitment to delivering the best patient experience by enabling patients to receive the right care in the right place at the right time.”

To ensure a seamless experience for patients and providers, SeamlessMD has been fully integrated with CHSLI’s Epic Electronic Health Record (EHR) and MyChart® patient portal. This allows patients to access it through MyChart, while providers can launch dashboards to monitor patient progress and view patient-reported outcomes from within the Epic EHR interface and leverage data to identify quality improvement opportunities.

“We are excited to be the first cardiac center to use SeamlessMD to educate, engage, and monitor patients undergoing TAVR,” said Kristin Pasquarello, Administrative Director of the Heart Valve Center at St. Francis Hospital. “This partnership is a testament to our cardiac center’s leadership in providing innovative patient care.”

About SeamlessMD
SeamlessMD is the #1 platform for patient engagement, remote monitoring, and virtual care used by health systems to engage, educate and track patients before, during and after a procedure. Evidence from academic medical centers has shown SeamlessMD to reduce hospital length of stay, readmissions, ER visits, SNF use, and costs. The platform is accessible for patients via smartphone, tablet or computer, and keeps patients on track from preparation through recovery with reminders, multimedia education, progress tracking, and Patient-Reported Outcomes data collection. Providers can access real-time dashboards to measure compliance and outcomes, intervene sooner for patients at-risk, and drive quality improvement across the organization.
