UAB Medicine: Since 2018, UAB has been a leader in leveraging digital care journeys to educate, engage and monitor patients pre and post-surgery - including being an early adopter of SMART on FHIR technology to embed remote monitoring dashboards for clinicians right inside the Oracle Cerner patient chart. This digital health approach has been particularly helpful to support UAB’s Enhanced Recovery Program (ERP), delivering evidence-based care pathways for surgery. A leader in both quality and research, this digital approach has led to improved clinical outcomes and lower costs which UAB has published in various journals:

Cornwall Community Hospital (CCH): In 2022, CCH became the first hospital in Canada to integrate its digital care journey platform with Oracle Health (Cerner) using SMART on FHIR technology. This integration allowed CCH to streamline patient enrollment and monitoring on SeamlessMD from right within the Cerner patient chart. Digitally guiding patients across hip, knee, shoulder, breast, colorectal and emergency general surgeries, over 1,700 patients have been supported. Highlights 91% feeling more confident before surgery and 86% feeling more confident at-home during recovery. Baystate Health: In 2018, Baystate Health not only became one of the first health systems to implement a Cardiac Enhanced Recovery After Surgery (ERAS) program, but it also became the first to leverage digital care journeys to deliver a digital Cardiac ERAS program to patients. Patients receive evidence-based reminders, education, symptom monitoring and PROMs data collection before, during and after major heart surgery. Beyond that, Baystate Health streamlined provider workflows through an Oracle Cerner SMART on FHIR integration that allowed nurse practitioners to remotely monitor patients through dashboards embedded in the patient chart. The results? Decreased hospital length of stay (↓0.9 days), readmissions (↓72%), observation stays (↓69%), ED visits (↓13%) and discharge to SNF (↓60%). Click here to read the case study.

Toronto East Health Network: TEHN’s digital care journey initiative has helped over 9,000 patients across surgical services, women’s health and care of the elderly. In 2020, TEHN was one of the first hospitals in Canada to digitally screen patients for COVID-19 symptoms leading up to surgery. Leveraging an integration between Oracle Cerner and their digital care journey platform, TEHN has streamlined enrolling patients into their digital health programs - allowing them to deploy digital care to patients at scale. The program is highly rated by patients, with 92% of patients saying they would recommend the program to others and 42% report using the program to avoid calls to the care team.
North York General Hospital: In 2022, NYGH rolled out digital care journeys to hip, knee and colorectal surgery, supporting over 3,000 patients since inception. To support patient adoption, the hospital also integrated their platform with their Oracle Cerner EHR, which enabled automation of patient enrollment into the program. Beyond the Cerner integration, NYGH also integrated its digital care journey platform with its digital front door, the North York Navigator app. The hospital has seen particular success for colorectal surgery, including lower length of stay and ED visits.
